Privacy Notice

Version 2026-10-17.1

Who receives your information

Smart-Tek Communications Inc. receives this inquiry. John X.F. Chen is your contact for this request and privacy questions. Use the contact details on this page. This notice applies to this incident-intake portal, not every Smart-Tek service or website.

What we collect

We collect the contact details, organization details, incident facts and supporting files you submit, together with a reference number, receipt time, selected language and your consent record. Do not submit passwords, authentication or recovery codes, private keys, payment-card data or unrelated personal information. Provide copies, not your only original evidence.

Why we use it

We use the information to assess your inquiry, contact you, discuss a possible investigation, preserve submitted records and administer authorized access to this portal. A separate scope and authorization are needed before investigation work. We do not use this portal to enroll you in marketing or sell submitted incident records.

Storage, access and service providers

Submissions and files are stored on a privately administered intake server. Authorized Smart-Tek administrators and reviewers may view and download them. Cloudflare provides the public HTTPS connection and processes requests and data in transit; information may be processed outside Canada. Access controls and encryption in transit reduce risk, but no online service can guarantee absolute confidentiality. Contact us before uploading especially sensitive material to arrange a suitable transfer method.

Cookies and preferences

This portal uses an essential session cookie for request security and management sign-in, and browser storage for theme and language preferences. Draft answers stay in the current tab until submission and are not stored in browser local storage. No advertising or analytics trackers are added by this application.

Retention and your choices

There is currently no automatic deletion schedule. Records are retained for review and any agreed investigation, subject to business, evidence-preservation and legal obligations. Contact John to request access, correction, withdrawal of consent or deletion. We will assess the request and explain any limits, including records we must retain. Withdrawal does not undo processing already performed. You may choose not to submit and contact us by phone instead.

Intake Terms

Version 2026-10-17.1

Purpose of this request

This form is for organizations or individuals seeking to discuss hiring Smart-Tek to investigate a suspected or confirmed cybersecurity incident. Submission is an inquiry only: it does not create an engagement, guarantee acceptance, establish a response time or reserve an investigator.

Separate scope, fees and authorization

Before any investigation, the parties must separately agree in writing on scope, fees, responsibilities, access permissions and evidence handling. Submission alone does not authorize Smart-Tek to access accounts, connect to systems, change settings, contact third parties or undertake investigation work. No payment is collected through this form.

Authority and accuracy

You confirm that you are authorized to share the information and records, including on behalf of the affected organization or person. Provide facts to the best of your knowledge, mark uncertainty honestly and avoid unrelated personal or confidential information. A reported indicator or normal system operation does not by itself establish whether a breach occurred.

Preserve evidence and use safe contact methods

Keep original records and submit copies. Avoid clearing logs or resetting equipment solely to complete this form. Do not send passwords, recovery codes or private keys. If an account or contact channel may be compromised, tell us a safer way to reach you. Ask John for an agreed method before transferring especially sensitive evidence.

Not an emergency response channel

This portal is not continuously monitored and submissions do not automatically send email alerts. For an ongoing incident, contact John by phone as well and follow your organization’s incident-response procedures. Do not wait for a form reply before taking time-critical action through your authorized response team.

Acknowledgment and language

By checking the consent boxes and submitting, you acknowledge these Intake Terms and the Privacy Notice in the selected language. The version, language and submission time are recorded. Available translations are intended to explain the same notice; contact John before submitting if any wording is unclear. Investigation outcomes cannot be promised at intake.
